Waiver and Release of Liability for Communicable Disease for In-person Attendees
ISITC has taken preventive measures to reduce the spread of communicable disease including, but not limited to, COVID-19 (“Communicable Disease”) or other bacterial or viral illness or disease, at its Conference. However, I acknowledge that ISITC cannot guarantee that I, my guests, or other individuals traveling with you will not be exposed to or contract a Communicable Disease at the Conference. Further, due to the nature of the facilities, services, and programs offered at this Conference, I understand that attending the Conference could increase my risk of contracting a Communicable Disease.

I voluntarily agree to abide by all applicable COVID-19-related requirements, advisories, policies, procedures, and protocols of ISITC, hotel, as well as the CDC, the City of Boston, and any other governmental authority for the duration of my stay.

The City of Boston has implemented an indoor mask mandate as well as vaccination requirement for those attending indoor events. Attendees are required to register in advance for the Conference. As part of the registration process, ISITC will ask if you are fully vaccinated against COVID-19. Proof of vaccination must be provided by all attendees, as well as their guests, during the check-in process. By providing proof of vaccination, I certify that I am providing truthful and accurate information regarding my vaccination status.

In light of the ongoing spread of COVID-19, individuals who fall within any of the categories below should not attend the conference. By attending the Conference, I certify that I do not fall into any of the following categories:

1. Individuals who currently or within the past ten (10) days have experienced any symptoms associated with COVID-19, which include fever, cough, and shortness of breath among others; or
2. Individuals who believe that they may have been exposed to a confirmed or suspected case of COVID-19 or have been diagnosed with COVID-19 and are not yet cleared as non-contagious by state or local public health authorities or the health care team responsible for their treatment.

I understand the risk that I may be exposed to or infected by a Communicable Disease by attending the Conference and that such exposure or infection may result in quarantine requirements, serious illness, permanent disability, and/or death. I further understand that the risk of becoming exposed to or infected by a Communicable Disease at the Conference may result from the actions, omissions, or negligence of myself and others, including, but not limited to, ISITC, its partners, affiliates, attendees, members, officers, employees, contractors, agents, representatives, sponsors, other participants, and any owners and lessors of premises used to conduct the Conferences (“Releasees”). I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, BOTH KNOWN AND UNKNOWN, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES, and assume full responsibility for my participation in the Conference.

In consideration of being allowed to participate in the Conference, I, for myself and on behalf of my heirs, guests traveling with me, assigns, personal representatives and next of kin, acknowledge and agree as follows:

A. I HEREBY RELEASE, COVENANT NOT TO SUE, DISCHARGE AND HOLD HARMLESS the Releasees, with respect to any and all liabilities, claims, penalties, suits, demands, judgments, costs, interests and expenses (including attorneys’ fees and costs) arising from or relating to the illness, disability, or death of myself (or any person who may contract a Communicable Disease, directly or indirectly, from me), as a direct or indirect result, in whole or in part, of a Communicable Disease, to the fullest extent permitted by law.
